General information

Age of insurance: 30 days old - 70 years old

For Maternity Care Benefit, the Insured Person is a female from 18 - 45 years old

Term of Insurance: 1 year, with auto-renew annually*

Premium: Calculated according to the age of the Insured at the time of issue of this Supplement and then adjusted for age at Policy Year Anniversaries**

(*) This Supplement is considered for renewal at each Policy Year Anniversary until the Policyholder requests to terminate the validity of this Supplement; or the Insured Person no longer meets the age-insured conditions set forth in these Additional Product Terms and Conditions; or Sun Life Vietnam discontinues the Supplemental Product or Plan(s).

(**) With the approval of the Ministry of Finance, Sun Life Vietnam may change the Premium of this Supplement. Sun Life Vietnam will notify the Policyholder at least 3 months before this change takes effect and the new Premium will be applied on the next Policy Year Anniversary.
